Here is a picture of the actual lesson plan I have written out for my two year old class. It is based on Teaching Strategies GOLD The Creative Curriculum® for Infants, Toddlers & Twos.

Volume 3: Objectives for Development & Learning: Birth Through Kindergarten explains the skills, knowledge, and behaviors that matter most to the continuing development and learning of very young children. A tool to help teachers observe children effectively, it offers 38 objectives for development and learning that are predictive of school success and aligned with state learning standards."
